CLAIM ADVICE

WHEN SUBMITTING A MEDICAL CLAIM PLEASE ENSURE THAT YOU DO THE FOLLOWING:

Attach original receipts to the claim form which must be completed and signed by the attending doctor. Before bringing the form to our office, fill in your personal information on the form and affix your signature. For Life Claims, call The Life Division 481-7822/23/24 and you will be instructed accordingly. For cheque inquiries, call 481-7800/01/02/03/04. TO REPORT A VEHICULAR ACCIDENT, FOLLOW THESE INSTRUCTIONS:

Give your name(s), address(es), contact number(s), vehicle number, date, time and location of accident. If your vehicle can be driven, without causing further damage, drive it to your home or auto body repair shop. If your vehicle is badly damaged and cannot be driven have it towed to your home or your auto body repair shop. Have a mechanic and or an auto body repair technician prepare an estimate for repairs next business day, please visit our office and present a current driver's license, registration card and or certificate of insurance, along with the estimate for repairs, if available. TO REPORT DAMAGE TO YOUR HOUSE OR BUILDING, FOLLOW THESE INSTRUCTIONS:

In the event of damage due to an insured peril, as far as possible, secure your house and/or contents. Never throw out damaged items before they are inspected. Never have any repairs done before all damages have been assessed. Have a qualified builder or engineer assess your damages and provide a detailed estimate. Bring this estimate to our office where our claims technician will assist you in completing the relevant form and advise you further. For further inquiries call 481-7817/18
